The assessment
engine
Structure and function, measured in the same week and read by one physician in one sitting. The MRI shows what your body is. The exercise, vascular and body-composition work shows what it does.
Six stages, front-loaded, over two days plus imaging. Everything measured here becomes the baseline against which any later intervention — in Langley or in Costa Rica — is judged.
The six stages
Stage 0
Pre-visit biology
Blood, at home, 10–14 days prior
68 distinct laboratory assays returning approximately 120 individually reported values — cardiometabolic, inflammatory, endocrine, organ-system, nutrient status, pharmacogenomics and guideline cancer screening. Every result is returned with its evidence grade.
Stage 1
Physiology block
Day one, morning — the signature
Maximal cardiopulmonary exercise testing with gas exchange: true VO₂max in mL/kg/min against age- and sex-matched normative data, ventilatory thresholds VT1 and VT2, RER, ventilatory efficiency and chronotropic response. Plus measured resting metabolic rate, 12-lead ECG at rest and on exertion, carotid-femoral pulse wave velocity with central aortic pressure, carotid intima-media thickness, endothelial function by peripheral arterial tonometry, DEXA with appendicular lean mass index and visceral adipose tissue, grip dynamometry, five-times sit-to-stand, gait speed, posturography and spirometry.
Stage 2
Sensory, cognitive, autonomic, sleep
Day one, afternoon
Computerised cognitive battery against normative data, pure-tone audiometry and speech-in-noise, acuity, contrast sensitivity and non-mydriatic retinal photography, validated olfaction testing, home sleep apnoea test reporting AHI and ODI with seven nights of actigraphy, heart rate variability and orthostatic response.
Stage 3
Imaging
Partner imaging centre
Whole-body MRI — head, neck, chest, abdomen, pelvis and spine, non-contrast, no ionising radiation. Coronary artery calcium scoring reported as an Agatston score against age- and sex-matched percentile, CT coronary angiography with FFR-CT where indicated, targeted ultrasound, and age- and sex-appropriate guideline cancer screening integrated rather than duplicated.
Stage 4
The interpretation consult
This is the product
One physician, the whole dataset, and the time to read it together — supported by Klooe, our interpretation software. You leave with a written healthspan assessment, a risk model, a ranked list of modifiable drivers, and a 12-month plan with named services and dates.
Stage 5
Re-assessment
Month 3, 6 and 12
Targeted repeat at month 3, the full physiology block at month 6, complete repeat at month 12. Same instruments, same protocols, so the comparison means something.
Whole-body MRI, honestly
A complete structural survey of the body without radiation.
The scan covers head, neck, chest, abdomen, pelvis and spine, and screens for solid tumours, aneurysm, spinal and joint degeneration, and organ and structural abnormality.
A meaningful fraction of scans surface something that turns out to be nothing. Following those findings has its own cost, its own procedures and its own anxiety, and any clinic that does not tell you so before the scan is selling you something else. What we do about it is the actual difference: a named physician reads your images inside your full physiological dataset, and there is a defined follow-up pathway for anything found — not a PDF emailed to a stranger.
We do not claim that screening imaging reduces mortality. The evidence does not support that claim. What we claim is structural information, read in context, with a pathway attached.
Treatment delivered in Canada
Everything below is delivered in British Columbia by a licensed physician using Health Canada–authorised products and licensed devices, and prescribed against measurement rather than symptom.
Physiology-led programming
- Exercise prescription written from measured VO₂max and ventilatory thresholds — individualised training zones, not generic heart-rate percentages
- Progressive resistance and sarcopenia-prevention programming prescribed against measured appendicular lean mass index
- Balance, gait and fall-risk programming for patients over 60
- Re-testing at defined intervals to confirm physiological response
Metabolic and nutritional medicine
- Nutrition protocol driven by continuous glucose monitoring, insulin sensitivity and body composition
- Structured weight and visceral-adiposity management
- Medically supervised time-restricted eating and periodic caloric restriction
- Micronutrient repletion — oral first, intravenous where clinically justified
Pharmacotherapy
- Lipid management targeted to ApoB: statins, ezetimibe, PCSK9 inhibitors, bempedoic acid
- GLP-1 and dual-incretin receptor agonists for obesity and metabolic disease
- SGLT2 inhibitors and metformin
- Blood pressure management guided by central pressures, not the brachial cuff alone
- Sleep apnoea pathway management and CPAP referral
Hormonal medicine
- Testosterone therapy for biochemically confirmed hypogonadism
- Menopause care: transdermal estradiol, micronised progesterone, vaginal estrogen
- Thyroid management
- Andropause and perimenopause assessment
Canadian hormone care uses Health Canada–approved formulations. Compounded preparations and secretagogues are delivered in Costa Rica.
Regenerative and procedural — autologous only
- Platelet-rich plasma: knee, shoulder, hip, tendinopathy
- Image-guided joint and soft-tissue injection
- Trigger point therapy
- PRP for hair restoration and dermal application
Device-based and environmental
- Whole-body photobiomodulation — multi-wavelength red and near-infrared LED at 633, 660, 810, 850 and 940 nm
- Intermittent hypoxic–hyperoxic training, supervised with continuous SpO₂ monitoring
- Mild hyperbaric oxygen therapy within the licensed pressure range of the installed chamber
- Infrared sauna and cold-water immersion, protocolised as contrast therapy
- Intermittent pneumatic compression and lymphatic therapy

Discuss Canadian assessment with a physician
Choose the clinic and the dates that suit you. Every course begins with an assessment, and we will tell you plainly if this is not the right intervention for you.
Interventions restricted under Canadian regulation are delivered at our Costa Rica clinic, under the same physicians and the same protocol.
